Add questions to a page
After creating the pages of the survey, you can start adding questions to the pages.
Add questions
Go to the page to which you want to add questions. Then click on ‘Add’ in the toolbar. You can choose from several categories. Each category has a set of question types.
Standard questions
- Open question: An open question can be answered with a paragraph of text. The field for the answer can be increased in size. Enter the the label and the variable name. The variable name will be used in statistical programs such as SPSS. Enter the actual question in ‘Title’. You can also add a description.
- Text field question: A text field question can only be answered with one line of text. The field for the answer cannot be increased in size. Enter the the label and the variable name. Enter the actual question in ‘Title’. You can also add a description.
- Date question: This type of question is answered with a date. This date can be in the past as well as in the future. Enter the the label and the variable name. Enter the actual question in ‘Title’. You can also add a description. Click ‘save’ to confirm.
Multiple choice questions
A multiple choice question has multiple options from which the participant can choose an answer. All options are displayed as a list. The desired answer can be selected by the participant. Create the question of type ‘choose one’ if the participant can only select one answer. Create the question of type ‘Choose multiple’ if the participant can select multiple answers.
Enter the the label and the variable name. The variable name will be used in statistical programs such as SPSS. Enter the actual question in ‘Title’. You can also add a description. In ‘Options’ you can enter all possible answers. If you run out of fields to enter options into, click on ‘add option’. To delete an option, click on . For questions of the type ‘choose one’, you can indicate whether other values are permitted as an answer as well. Click ‘save’ to confirm.
Selection questions
Selection questions are similar to multiple choice questions, but they are displayed differently on the survey. For a selection question with one choice (Choose one), a drop-down menu will be used. The desired answer can be selected in the drop-down menu. For a selection question with multiple selection options (choose multiple), the options can be selected by clicking on them. To select multiple options, the Shift key (Windows) or Command key (Mac) must be pressed while selecting the options.

Matrix questions
A matrix question consists of multiple sub questions that can all be answered with the same set of answer options. Create the question of type ‘choose one’ if the participant can only select one answer per sub question. Create the question of type ‘Choose multiple’ if the participant can select multiple answers per sub question.
Enter the the label and the variable name. Enter the actual question in ‘Title’. You can also add a description. In ‘Options’ you can enter all possible answers. If you run out of fields to enter options into, click on ‘add option’. To delete an option, click on . Enter the sub questions under ‘Sub questions’. To delete a sub question, click on . click ‘save’ to confirm.
Score questions
You can create score questions the same way as multiple choice questions. The difference between the two is that for score questions, you can assign scores to the answer options. You can read how to do this in this article under ‘options’.
Page content
Instead of a question, you can also add a text block to a page. You can give the text a title and you can enter a description. This can be used for extra instructions or explanatory notes about the questions that follow. Click on ‘save’ to confirm.
Overview of questions
All questions you created are displayed in a table. The table contains the number (#) of the question, the question, title and type of question. In this table, you can make a question required by selecting ‘required field’. You can also delete the question () and move the question to another page of the survey () under ‘Options’.
The questions are also displayed in the Survey tree under the page on which they were placed.
Edit order
Click on ‘Edit order’ to edit the order of the questions. Drag the questions into the desired order and click ‘save’ to confirm.
